PROGRAM PERIOD: The Motiva^® Influencer Special Offer (the “Program”) is offered by Motiva USA, LLC (“Motiva”) and applies to patients that receive a Qualifying Surgery with Motiva Implants^® (“Qualified Surgery” as defined in Section 3) between May 05, 2026, and July 05, 2026 (the “Surgery Window”).  The Program Period includes a “Surgery Window” and a “Redemption Window” as stated in the chart below which will be used by Motiva to determine eligibility for the offer described herein:

ELIGIBILITY: Open to legal residents of the United States at least 22 years old at the time of their Qualifying Surgery who have received their Motiva Implants® in any of the fifty (50) states, the District of Columbia, or Puerto Rico (“Participant”). Eligibility for the Program is contingent on a rebate code being sent by Motiva to a Participant who undergoes a Qualifying Surgery during the Surgery Window of the Program Period with a surgeon administrating the Program using Motiva Implants^®. Participant must claim rebate code prior to surgery date. If rebate code is claimed after surgery date, the Participant is not eligible for the program and their submission will be denied. Licensed health care professionals, employees of licensed health care professionals and Motiva employees are excluded from participation and are not eligible to receive benefits from the Program.  Offer valid once per patient. Offer cannot be combined with another Motiva^® influencer special offer promotion for $250 gift card.

REDEMPTION OFFER AND PARTICIPATION: A Participant who views social media content from a Motiva^® Influencer may visit the influencer’s account and click on the “Linkinbio” reference to the “Motiva Influencer Special Offer” where they will be provided with a form for submitting certain contact information to Motiva. Once such information is submitted, Motiva will then email the Participant a unique influencer rebate code, Program details and instructions on how to use the code for a rebate on Motiva Implants^®. During the Surgery Window (as defined in the chart in Section 1) a Participant who undergoes a breast augmentation surgery performed by a  licensed Motiva^® surgeon in which two (2) Motiva^® breast implants are implanted in accordance with the implant’s directions for use (the “Qualifying Surgery”) will be eligible to receive a rebate in the form of one (1) $250 gift card from Motiva. Reconstruction surgeries, or any surgeries conducted with Motiva Implants^® that are reimbursable, in whole or in-part, by any federal healthcare program (including Medicare, Medicaid, and TRICARE) or by any public or private insurance, are not qualifying surgeries under this Program and not eligible for a rebate/gift card. Upon completion of a Qualifying Surgery, the Participant must then register their Motiva Implants^® within the Redemption Window (as defined in the chart in Section 1) on the MotivaImagine^® application where they will be prompted to enter their previously e-mailed unique influencer rebate code.  Motiva will redeem the rebate in the form of a gift card after receiving the code from the Participant, subject to the Verification process described in Section 4 below and after the eligibility of the Participant is confirmed.
